Overview of the Role We're seeking an experienced and compassionate Home Health Nurse to join our team as a Hospice Case Manager in the Youngstown territory. This is a full-time, 1st shift position that offers a competitive salary, attractive benefits, and a $10,000 sign-on bonus. As a Hospice Case Manager, you'll play a critical role in delivering high-quality patient care, collaborating with our care team, and making a meaningful impact on the lives of patients and their families. Key Responsibilities Performs comprehensive patient assessments and collaborates with the care team to develop and implement individualized plans of care. Makes referrals to other hospice care disciplines as needed and documents rationale for not doing so. Maintains accurate, timely, and detailed patient care progress records according to care center policies. Records pain/symptom management changes and outcomes as appropriate. Teaches patients, families, and caregivers essential patient care techniques. Regularly communicates with clinical managers and care teams to ensure seamless care coordination. Performs on-call responsibilities and provides on-call services to patients and families as assigned. Other duties as assigned. Essential Qualifications One year of nursing experience, preferably in home health or hospice care. Current RN license, specific to the state(s) you are assigned to work. Current CPR certification. Valid driver's license, reliable transportation, and liability insurance. Preferred Qualifications Experience working in a hospice or home health setting. Strong clinical skills and knowledge of hospice care principles. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Compensation, Perks, and Benefits We offer a competitive salary range of $75,000 - $85,000, depending on location, job-related knowledge, skills, and experience.
