Job Overview

Key Responsibilities
? Manage a designated caseload in accordance with directives from Care Management Leadership.
? Engage with enrollees at predetermined intervals, offering supplementary support as required.
? Conduct in-person visits in diverse environments, including enrollees? residences and healthcare facilities.
? Evaluate enrollees upon their enrollment and periodically to ascertain needs, obstacles, and potential areas for care enhancement.
? Coordinate case management tasks, prioritizing quality, timeliness, and adherence to industry regulations.
? Collaborate with Senior Care Managers to prioritize cases, establish objectives, and tackle potential risks or utilization issues.
? Seek advice from more experienced team members for specialized care requirements.
? Ensure precise and timely documentation of enrollee interactions and assessments, in line with organizational standards.
? Adhere to confidentiality protocols pertaining to enrollee information.
? Engage in discharge planning to connect enrollees and their families with suitable healthcare resources.
? Educate enrollees and caregivers about preventive health services, highlighting the importance of compliance with recommended care practices.
? Utilize comprehensive knowledge of target population conditions and established care management methodologies.
? Review provider reports and facilitate the coordination of follow-up care as necessary.
? Assist enrollees in the effective use of assistive technology, medication management, and self-care techniques within the defined scope of practice.
? Support enrollees in managing care transitions, such as provider changes or transitions between care environments.
? Process authorizations for services that require approval from Care Management personnel.
? Communicate effectively with enrollees, their families, and healthcare providers to foster understanding and collaboration.
? Maintain professionalism and a customer-centric approach in all interactions.
? Apply strong organizational and time management skills to balance workload effectively.
Required Skills
? Exceptional verbal and written communication abilities.
? Familiarity with medical terminology and healthcare delivery systems.
? Proficient in Microsoft Office and other pertinent computer applications.
? Excellent organizational competencies and the ability to interpret and apply departmental policies.
? Strong decision-making capabilities and sound judgment.
? Capacity to perform efficiently under pressure and meet deadlines, ensuring outstanding customer service.
? Experience in participating in professional multidisciplinary meetings with internal and external teams.
? Bilingual (Spanish-speaking) preferred.
Qualifications
? 3-5 years of relevant experience in care management, managed care, or community resource settings.
? Active licensure in Washington, D.C. as a Licensed Independent Clinical Social Worker (LICSW) or Registered Nurse (RN).
? Certification as a Case Manager is highly desirable.
? An Associate's degree is mandatory; a Bachelor's degree is preferred.
